Design to create an eye-catching A4 booklet 11,000 words

I have an 11,000 word document which I would like created into an online and print version A4 booklet. The style I am looking for is below. It should have pictures, image, different font sizes, in black and white, boxes, circles, bold words, title pages etc. Needs to be very ENGAGING, not boring corporate.

I have attached 3 pages of the word document, and would appreciate a mock up of this so that I can see the style. I just need a single page to get an idea of your work.
Price-wise I don't know what is reasonable for this but am willing to offer £150 and we can discuss. There are 42 pages in a word document 11,000 words, but I am writing with a big font and widely spaced apart. I think that the final document should be either 16 or 20 pages. the online version can be any number of pages but print has to be multiple of 4.
