Create an online survey

I need help in developing a online survey. I have written up the survey questions in a Word document, and need help to create a survey using an online tool called https://app.startquestion.com/ (I will share my account credentials). The work will include structuring the survey and formatting each question appropriately (choosing the question type, e.g. open ended, multiple choice etc.).
The survey is ~15-20 questions and I would expect it would take 2-3h to complete (including one feedback iteration).
