I currently have a very basic online form that allows a user to enter the contact details of a business, including their address, and then produce a QR barcode that contains those details.

The QR barcode generation is fine, there is no changes needed there.
The work that is needed is to incorporate Google Places API so that when the user starts typing an address, the Google API will display matches and then once selected put the address details in the appropriate fields.
It is also a requirement to make the form ‘mobile friendly’ with an emphasis of using it on an iPad tablet.
CSS styling is required for the form and there is no requirement to use existing css. If required the form can be completely rewritten, as long as the QR generates the same way as it currently does.
Google API documentation can be found at: https://developers.google.com/maps/documentation/javascript/examples/places-autocomplete-addressform#maps_places_autocomplete_addressform-html
FTP access and Google API key details will be provided to successful applicant.
Applicants who supply examples of similar work will be considered first.
